Support Coordination

Support Coordination is a Capacity Building Support that assists individuals get the most out of their NDIS funds and build capacity.

Our experienced and qualified support coordinators can assist elegible participants with:

Cooordination of supports; by connecting suitable service providers
understanding your NDIS plan including access to My place portal and funding budgets
referrals to qualified allied health professionals
preparation for NDIS planning and review
ensuring the implementation of plan whilst building capacity
offering choice and control in selecting suitable providers and including you in every step
